Physical Therapy Evaluation Form
A Physical Therapy Evaluation Form is a document used by the physical therapists to evaluate the current condition of the patient who has injuries or other health-related conditions. This form is important because this will determine if the patient needs more treatment or how the treatment process will undergo. This Physical Therapy Evaluation Form contains form fields that ask for the client's personal details, occupation, and marital status.
This form template also asks what are the patient's complaints, how did he/she get the injury, past medical history, and the condition of the treatment or the previous therapy. This also checks the client's responsiveness, orientation, safety awareness, range of motion, and activities that worsen the condition. This template is using the Input Table to know the range of motion status of the upper and lower extremities.
