I tested your form using the code from your screenshot in the Spreadsheet to Form widget, and I received a different error: "Invalid Code."

This means the widget is working on my end, but the code entered doesn't match any in the uploaded file. Could you clear your browser cache or try using a different browser. If the issue persists, remove your Excel file from the Spreadsheet to Form widget and re-upload it.
