Buenos días les comento que al llenar el formulario con nombre "Experiencia SIVA" me figura el siguiente mensaje
Me pueden ayudar en validar si sucede algo con mi cuenta

Hi Tobar_ctobar,
I’ve reviewed both the form and the associated account, and everything appears to be functioning correctly on my end. To further troubleshoot, I also cloned the form and tested it myself and I was able to open it and complete a submission without encountering any errors. This suggests that the form itself is working as expected. Check out my results in the screencast below:
