Now, coming back to your question, unfortunately it is not possible to export submission data as an Excel file with a specific format you want to use, just like the screenshot you shared with us. If a submission data is exported as an Excel file from a form's Tables, it will be downloaded in a default format, showing the column/field names, and their respective entries. Check out the screenshot below:

To follow the format you want to use for the submissions, you may need to download them first and then edit the entries externally. You can easily download your data from Jotform Tables as a CSV, Excel, or PDF file. Let me show you how:

1. On your My Tables page, hover over the form whose data you want to download, and click on View Table.
2. On the upper-right side of the page, click the on Download All button. 
3. Now, select Download as CSV, Excel, or PDF to download your data. For your specific purpose, you can select Download as Excel for that.
