    Gracias.

    Hi Carles

    Thanks for reaching out to Jotform Support. Our Spanish Support agents are busy helping other Jotform users right now, so I'll try to help you in English using Google Translate, but you can reply in whichever language you feel comfortable using. Or, if you'd rather have support in Spanish, let us know and we can have them do that. But, it might take a while until they're available again.

    Now, coming back to your question, unfortunately, there's no widget available to allow the form fillers to add more upload fields to the form. However, we have the Configurable List widget, which allows them to add more incidents as needed, since it allows adding more rows of fields where they can input the incident details with the press of a button. Let me show you how to set it up:

    1. In Form Builder, click on the Add Element menu on the left side of the page.
    2. Under the Widgets tab, drag and drop the Configurable List widget into your form or just click on it to add.
    3. In the menu on the right, customize the Fields Configuration section according to the syntax below.
    {label} : {type} : {placeholder}

    As for the images, you can use the File Upload element, as it allows multiple file uploads:

    1. In Form Builder, open the Form Elements menu.
    2. File the File Upload element, then add it to your form.
