Cuenta suspendida

Me bloquearon la cuenta por pishing y el formulario no tiene nada que ver son eso, tengo personas esperando el formulario y o pueden acceder, necesito una solución de manera urgente por favor.

When I checked your account, I saw that it was suspended due to your form titled Datos Pagina Web Tecnolar offering a fake service & collecting login information. If you believe that your account was suspended in error, we'll need you to send the following information to reviewers@jotform.com with Thread #21686871 as the subject:

1. The URL where the form is used.
2. A link to your website or proof of affiliation with a legitimate institution.
3. Any supporting documentation that can validate your claims.

Sending the email from your business email address, if you’re affiliated with a legitimate institution, would fasten the process. We also need you to change your registered email to a corporate one, if you have one, to avoid getting flagged again by our phishing detector.
