Information is duplicating

Greeting, hope you're doing very well.

We're having some trouble with forms.

Sometimes, although we're adding the boxes in the correct format, it may occur that settings change, and the boxes get mixed up, and the information duplicates.

From my end, I just fixed this issue, but just wanted to let you know that it happened a few times.

I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. I have not experienced any duplicating field. Check out the screnshots below to see my results:

Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
