How to add an intro to my form?

and include an intro, or have a pop up prior to the form appearing that has an intro to the form?

Thanks for reaching out to Jotform Support. It's not possible to create a pop-up on form load, but we can have a page at the start of your form that could server as the introduction. Let me show you how:

1. In Form Builder, click on the Add Element button on the left.
2. Click and Drag the Page Break element to the top of your form.
3. Now, you can customize the initial page using the fields such as the Paragraph field, image and header.
