I want to be able to add and format text above some of the questions

and include an intro, or have a pop up prior to the form appearing that has an intro to the form?

    Hi Josayef,

Thanks for reaching out to us for help. I checked your form and see that you have used Heading for the texts in your screenshot. You can use the Paragraph Field instead so you can format the texts to your liking. Let me show walk you through it:

    1. In Form Builder, click on the Add Element button in the top-left.
    2. Under the Basic tab, look for Paragraph and drag and drop it to your form.
    3. Click on the Pencil Icon beside it to open the Text Editor.
    4. Put in your text and design it to your liking.

That's it. As for your other question, I moved it to a separate thread to keep things tidy, and someone from our Support Team will help you out with it here.

Let us know if you need any more help.
