They load the screen for about 1/2 a second and then glitch to white. I have cleared the cage, restarted my phone, restarted the app, and more.

Thanks for reaching out to Jotform Support. I understand that your forms load briefly and then the screen glitches to white, and you have already tried clearing the cache, restarting your phone, and restarting the app. It looks like the issue only happens on your mobile device, as I was able to view your form without issues on a Web Browser. Can you try force syncing your data to your Mobile App? Let me show you how it's done:
1. On your My Forms page, tap on the Gear icon in the top right corner of the screen to open Settings.
2. While in Settings, scroll down and tap on the Refresh icon in the Sync Your Data section.
If you want the app to refresh and get the latest data without any issues, you should enable the Background App Refresh feature for the Jotform App in your phone settings. In case the issue persists, you can also try reinstalling Jotform Mobile App to see if it makes any difference.
