Hi! I'm currently creating a survey and used a template. The title page of the original template is not available for me to remove. Please advise on how I can have the survey start with our custom survey title.

Hi Leann,
Thanks for reaching out to Jotform Support. Just to make sure, are you talking about this form? If yes, I can see that the form's title has been updated as needed. But, I believe you are talking about the form's Welcome page, which is the first one to show up once it's initially accessed. Check out the screenshot below:
If yes, and by default, the Welcome page is hidden from the form when accessed through Form Builder for those using the Card Form layout, so it's really easy to miss. But it's really easy to fix. Let me show you how:
1. In Form Builder, hover your mouse over the blank section above the first card named, Self-Care Personality Style.
2. The Edit Welcome Page button will show up, and then click on it.
3. From there, you can edit its Title and Subtitle, whatever you desire they should be.
5. In the Welcome Image Properties window on the right, click on Upload File and add your desired image for the logo. All changes are saved automatically.
