Hi Tabitha,
When I cloned your form and tested it, I was also not able to receive the Notification email. After further checking, I've found that this is because you have not checked the When a submission is received from the email settings. Check out my screenshot below:
You need to check that feature to be able to receive the Notification email. Let me show you how to do this:
- In Form Builder, click on Settings on the orange navigation bar at the top of the page.
- Select Emails from the left side window.
- Click on the Pencil icon of the Notification.
- From the Advanced tab, tick on When a submission is received under the Send Email Automatically.
- Click on Save at the bottom of the page.
