Collects customer info (name, order, email) and issue details

Hi Will,

In order to collect customer information. You can use the Full Name element to capture the name, the Email field for the email address, and the Product List to get orders. Let me show you how:

1. In Form Builder, click on the Add Form Element menu on the left side of the page to see the list of elements and widgets.

2. Under the Basic, Tab, scroll down the list, and then drag and drop the fields to the form.
