We need a customer service form solution that:
- Collects customer info (name, order, email) and issue details
- Allows photo uploads for damaged products
- Routes submissions to different email addresses based on product type and customer location
- Stores all submissions in a database or spreadsheet
- Sends automated email notifications with submission details to appropriate teams
- Manages product replacement requests with minimal manual intervention
