Boa tarde, Foi idenficado que em um determinado de pergunta quando a pessoa responde, apenas aprece a resposta no fomulário enviado por e-mail

Or, if you'd rather have support in Portuguese, let us know and we can have them do that. But, keep in mind that you'd have to wait until they're available again.
Now, let me help you with your question. I cloned your form and tried to replicate the issue, but all the questions appears in the Notification Email. Check out my result in the screenshot below:
If the Notification Emails are not displaying all the questions, try to delete the existing one and create a new Notification Email. Check the steps and screencasts below on how to update your Notification Email.
Start by deleting the existing Notification Email:
- In Form Builder, click on Settings.
- Select Emails.
- Hover on your existing Notification Email.
- Click the Trash icon.
Then create a new Notification Email:
- On the same Emails settings page, click the + Add email button.
- Choose Notification Email.
- Check the email content.
- Click the Save button to save the email.
Once you've done that, all the questions in your form will appear in the Notification Email.
