Necessito que os usuários façam autenticação (usuário e senha) para acesso ao formulário.

Como fazer isso?

With that being said, let me assist you with your inquiry. Unfortunately, forms cannot be used to ask login credentials from users as it violates our Terms of Use. Your account might get suspended if the system detects that you have fields asking for these credentials.

As a workaround, you might consider enabling password protection in your forms. Please see this guide on How-to-Password-Protect-a-Form.
