Now, coming back to your question, I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. Let me show you how to fix it:
1. In Form Builder, select the Ticket Purchases widget.
2. Click on the Wand icon to open Widget Settings.
3. In the menu that opens on the right, scroll down to the bottom and click on Update.
And that's it. Just repeat the same steps to the other widgets and that should fix the issue in your form.
