Criar uma Avaliação de Escala de Formulário

Estou criando um formulário de avaliação em escala - preciso que conste 2 perguntas e a escala de 0 a 10. Minha intenção é exemplificar 2 situações na perguntas e colocar uma escala de 0 a 10 para que apontem o nível de maturidade dos processos pesquisados. É possível?

Now, coming back to your question, the Scale Rating field makes it easy to add a scale from 1 to 10 or whatever range you like. Let me show you how:

1. In Form Builder, click on the Add Form Element menu and click on Scale Rating to add one to your form.
2. Under the Basic tab, scroll down to Scale Rating, and drag and drop a Scale Rating element to your form.
3. Click on the Gear icon on the right side of the element to open the Scale Rating Properties.
4. Click on the Options tab and select your Lowest Rating Point and Highest Rating Point from their respective dropdown menus, and that’s it. You're done.

Thank you very much for the quick response. I managed to add the scale, but my question is how to include two questions for the respondent to indicate their level of management maturity. See the example below: Note that there are two questions and a scale to indicate the level of management maturity.

I'm not exactly sure if I got your workaround correctly. But can you confirm if you want to insert two questions in a single scale rating? If so, it's not possible. You can include only one question per Scale Rating element. To guide users, add rating labels (e.g., Best and Worst) so they can interpret the scale correctly. If you have two questions to rate, add two separate Scale Rating elements to your form, one for each question. If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?

You can only add one question per Scale Rating element. If you have two questions, just add two separate Scale Rating elements. If you have multiple questions you'd like to group, you can create a Likert Scale. Instead of using the Scale Rating element, you can use the Input Table element to display multiple questions with a shared rating scale. Check out the sample below:
