Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Forma that’s breaking our Terms of Use by collecting login credentials.

If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.

Once we hear back from you, we'll be able to move forward with this.

eu notei que essa opção pode ser usada para outros usuarios porem o meu encontra-se bloqueado. Trabalho em uma empresa de contabilidade que nescessita deste dados caso o cliente queira abrir um cnpj em seu nome, se eu colocar essa opção como não obrigatoria poderia ajudar a revogar a suspensão da minha conta?

We understand that you need to collect this information, however, Jotform does not allow collecting of ID or any login credentials. Making it unrequire will still suspend the account.

Let us know if you'd like to proceed with deleting the fields.
