Do nada minha conta foi desativada, por gentileza reative. Não fiz nada demais, apenas baixei os formulários e estava editando o formulário incluindo novos campos e a tela deu refresh e bloqueou.

Now, coming back to your question, your account was suspended because you’ve got a form named Briefing inicial - Social Media that was tagged as phishing. We understand that this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use. I did check your form, and there is a one offending field which is collecting user and password for the social media. Keep in mind that, on Jotform you can't collect the username and the passwords, they are considered as a personal sensitive information. Let me share the screenshot of the offending field below:
