As tabelas de envio não estão carregando corretamente.

Olá, estamos com um problema ao tentar visualizar as tabelas, ao clicar em visualizar respostas na tabela, a tela fica como nas imagens abaixo, testei em várias tabelas e o erro é o mesmo. Ao clicar em editar formulário, também foi apresentado alguns erros, abaixo segue as imagens.
