    Now, coming back to your question, I checked your account and I noticed the account was suspended due to collecting sensitive information such as login and credentials. Please note that the collection of sensitive information such as credit card information, and login credentials, is violating our Terms of Use.

    To reactivate your account, we need your permission to delete the offending form. Please confirm so we can proceed with the deletion and reactivate your account.

    It is acceptable to request an access code to conditionally show/hide fields on your form. However, you should avoid using "password" as your label to avoid being flagged by our phishing detector. Please replace it with something similar, such as "access code," as other types of passwords or user credentials will not be collected.

    Once we hear back from you, we'll be able to help you with this.

    Hi Igor,

    I've successfully deleted the offending fields in your forms and activated your account. You should be able to log in now. I just removed the field name password and word password on pages 4.
