Minha conta foi suspensa por erro de configuração do formulário que violou os termos. Gostaria de recuperar a conta e corrigir os erros.

Now, coming back to your question, your account has been suspended due to a form named Gestão de | L2A Consultoria that has violated our Terms of Use. Please remember that collecting login credentials is not allowed on Jotform. I've removed the offending fields and reactivated your account. You should be able to login now.
