COMO RESOLVER PROBLEMA DE PHISING?

    Olá, bom dia.
    Espero que esteja bem.

    Ao tentar acessar minha conta no jotform, apareceu a tela que anexei.

    Acredito que isso tenha acontecido, pois coloquei um campo de "senha", mas era senha apenas para limitar o acesso das pessoas ao meu forms, ou seja, somente aquelas que possuem a senha poderiam responder o meu forms. Se eu solicitar que a pessoa insira o código ainda terei esse problema?

    O forms que criei não pede nenhuma informação sensível, peço que revisem e corrijam, pois já mandei imprimir o QR Code desse forms.

    Now, coming back to your question, your account was suspended because you’ve got a form named Form that’s breaking our Terms of Use by collecting login credentials.

    If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.

    Once we hear back from you, we'll be able to move forward with this.
