    As I can check from the backend there is no form present in your form and your account is under the Starter plan. Can you please let us know if you are trying to upgrade your account? If so, do let us know we will forward your request to the related team.

    Or are you filling out the form in which you need to make a payment but you are not able to do it? In this case I would suggest you contact the person who provided you the form. They would be able to help you with this.

    Once we hear back from you, we'll be able to move forward with a solution.
