    Hi saverianisalerno,

    When I checked your account, I noticed that it was suspended due to your form titled Clone of the Saverians Salerno Registration Form includes fields to collect sensitive information, which violates our Terms of Use. For us to lift the restriction, we'll need you to send the following information to reviewers@jotform.com with Thread #31218421 as the subject:

    1. The URL where the form is used.
    2. A link to your website or proof of affiliation with a legitimate institution.
    3. Any supporting documentation that can validate your claims.
    4. A valid government-issued ID card, such as a driver's license or passport, along with a clear, forward-facing selfie for verification.

    I checked the documents you sent and found that your business is legit, so we’ve reinstated your account. You should be able to log in now.
