Now, coming back to your question, it's easy to calculate the total score after assigning calculation values on each option. Let me show you how to do that:

1. In Form Builder, click on Add Element+ on the top-left of your screen.
2. Click on Widget tab, then click or drag-and-drop Form Calculation to add it into your form.
3. A new configuration will appear on the right side of your screen. Click on Add Field and select the field you would like to add. After that, add plus (+) sign, then repeat to add the other fields.
4. Once you have finished configured this, click on Save, and you are good to go.
