    Now, coming back to your question, I understand you're having issues filling out your form due to the layout being changed.

    It looks to be a form containing a single Signature Field element.

    How exactly did the form behave for you prior to this issue? I've accessed a cloned version of the form on an iPhone mobile device and don't notice any issues.
