Un formulaire ne fonctionne plus

Bonjour,

Ce message apparaît sur les paramètres d'un de nos formulaires. Nos usagers ne peuvent plus le remplir.

Est-il possible de m'expliquer comment remédier à ce problème ?

Let me get back to your question. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:

In Conditional logic, the Missing field is showing due to some fields being deleted from your form. So, those fields are shown as missing fields. You can restore the deleted fields in your form by using Revision History. Let me show you how:

1. In Form Builder, click on All changes saved text below the Form Title at the top.
2. In Revision History menu you can see the changes you have made.
3. Choose a Revision and click on Revert to restore that revision.

Pouvez-vous m'expliquer comment faire pour que les réponses obligatoires (avec un *) soient filtrées en fonction de la formule choisie par nos usagers ?

Pour le moment, ils sont obligés de cocher des réponses pour toutes les propositions de visites que nous faisons. Nous aimerions qu'ils ne répondent qu'en fonction du type de visite qui aura été choisi.

Pour être plus claire, nous proposons :

-Visite clé en main

-Atelier

-Sur-mesure

-Visite en autonomie

Comment faire pour que si l'usager choisi une "visite clé en main", il n'ait pas à cocher une thématique dans "visite autonomie" ?

Le même problème est présent dans le formulaire "Petite enfance".

Hi Sandra,

Thw Show/Hide conditional feature lets you show questions based on the answers that people gave. Let me show you how to set it up:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. Click on Conditions on the left of the page.
3. Now, click on Add a New Condition and then click on Show/Hide Field.
4. Set the conditions like you see below:

• IF: Do you have a dog or a cat? (this is the question that has the choice that the form submitter needs to select)
• STATE: Is Equal To
• VALUE: Dog (the first choice)
• DO: Show
• FIELD: What is the name of your dog? (the follow-up question that you want to show)
