MEDIA RELEASE

I recognize and acknowledge that the YAC Foundation may record my  participation and appearance in ACTIVITY on any recorded medium (including, but not limited to video, audio, photos) for use in any form. I authorize such recording and release YAC Foundation to use my name, likeness, voice, and biographical material to exhibit or distribute such recordings in whole or part without restrictions or limitations for any educational or promotional purpose.

RELEASE OF LIABILITY

As parent/guardian of the above student, I hereby agree to release and exempt YAC Foundation, YAC Sports, Camarillo High School, City of Camarillo, Good Shepherd Church, Moorpark College, Oxnard Union School District and any other agencies its officers, agents, employees or representatives from any and all claims, demands, or causes of action whatsoever, including all cost and legal expenses incurred, if any, arising out of any damage, loss or injury resulting from or concurrent with an activity on the premises, or which may result from a condition created or maintained on the premises. Due to Covid-19 YAC is following all protocols and preparedness plans for business, including social distancing, sanitizing of equipment and surfaces, providing temperature readings and sanitation stations to ensure safety of all staff and members. By allowing your child to participate in any activities you are agreeing to adhere to all rules and regulations set forth. I hereby certify that the above information is correct.

YAC ELITE CHEER GYM WAIVER, AKNOWLEDGEMENT OF RISK & MEDICAL AUTHORIZATION

As legal guardian of the above child(ren), I recognize that potentially severe injuries, including but not limited to permanent paralysis or death can occur in sports or activities involving height or motion, including but not limited to cheerleading, stunting, gymnastics, open gym, tumbling, & dance. Being fully aware of these dangers, I voluntarily consent to the aforementioned child(ren) participating in any and all of YAC Elite Cheer, Young Athletes for Christ Inc. programs and activities and accept all risks associated with that participation. Parents should make their children aware of the possibility of injury and encourage their children to follow all safety rules and the coaches’ instructions. I am also aware that the gym area is for participants only and that if I enter the gym, I am doing so at my own risk.

In consideration of allowing my child to use these facilities, I on my own behalf and the behalf of my child, hereby assume all risks associated with the activities mentioned above and agree to hold YAC Elite Cheer, Young Athletes for Christ Inc., its staff or representatives harmless from any and all liability, caused of action, debt, claims or demands of any nature whatsoever which may arise in connection with participation in cheerleading, stunting, gymnastics, open gym, tumbling and dance or in the course of any exhibition, competition or clinic in which he/she may participate or while travelling to or from the event. In the event of an emergency, I hereby release YAC Elite Cheer, Young Athletes for Christ Inc., staff or representatives to render temporary first aid to my child or children in the event of any injury or illness and if deemed necessary by YAC Elite Cheer, Young Athletes for Christ Inc., staff or representatives to seek medical help, including transportation to any health care facility or hospital, or the calling of and ambulance for the said child(ren) should YAC Elite Cheer, Young Athletes for Christ Inc., staff or representatives deem this to be necessary. Additionally, I hereby agree to individually provide for all possible future medical expenses which may be incurred by my child because of any injury sustained while participating at or for YAC Elite Cheer, Young Athletes for Christ Inc. I also affirm that I now have and will continue to provide proper hospitalization, health, and accident insurance coverage which I consider adequate for both my child’s protection and my own protection.
