Now, coming back to your question, can you tell us more about the issue? Did you mean you're unable to add the Spreadsheet to Form widget to your form? If yes, can you share with us the URL of the form so we can check what's going on? Here's where to find your form URL:

1. On your My Forms page, hover your mouse over the form and click on the More dropdown on the right side of the page.
2. Under the Publish column, select Publish Form.
3. Click on the Copy Link button.

Also, if you can take a screenshot of what’s happening, that will help us to better understand what’s going on. We have a guide about how to do that here.
