Hello,
I need some assistance connecting JotForm with Square. I already have everything set up in Square, but I’d like to add forms that clients can easily fill out. I tried setting things up, but I don’t think I did it correctly.
Ideally, I’d like to use JotForm for all client interactions (intake forms, SOAP notes, etc.) while keeping Square as the only platform for processing payments. I would also like to create a form that includes two or three different sections/forms, with the option for clients to download them if needed.
Could you please guide me through the setup or let me know the best way to structure this?
Thank you for your help,
Alejandra

Hi Alejandra,
Thanks for reaching out to Jotform Support. Let me help you with integrating Jotform with Square and structuring your forms.
To connect your Jotform with Square for payment processing while using Jotform for client interactions, here’s how you can set it up:
- In Form Builder, click the Add Element button on the top-left side.
- Search for Square under the Payments tab and add it to your form by clicking or dragging it.
- In Payment Settings, select your mode: Test Mode for sandbox testing or Live Mode for real payments.
- Click Connect to link your Square account and allow permissions as requested.
- Configure options such as Business Location, Currency (auto-set based on your Square account country), Payment Type (products, subscriptions, donations, or user-defined amounts), and Payment Methods (credit card, Apple Pay, Google Pay, etc.).
For your requirement to have a form with multiple sections (like intake forms, SOAP notes, etc.) and allow clients to download them, you can create a single form with multiple pages or sections using the Form Builder’s page break or section collapse elements. This way, clients can fill out different sections in one form.
To enable clients to download the completed forms, you can set up an autoresponder email with a PDF attachment of their submission or use the PDF Editor to customize the PDF format.
If you want to reuse the Square payment setup across multiple forms, you can use Jotform’s Reusable Payment Connections feature to save the Square integration once and apply it to other forms without reconnecting each time.
Let me know if you need detailed steps on creating multi-section forms or setting up downloadable PDFs.
Give it a try and let us know how it goes.
