The form address works for awhile and then will show form missing. There is nothing wrong with the setup and it is not trashed or archived

Since you have already checked your internet connection, cleared your browser cache, tested the form on different browsers and devices, and confirmed that the form settings are correct, the next step would be to check your account's My Workspace page at https://www.jotform.com/workspace/. Please see if the form is visible there.
On the My Workspace page, click on Type at the top and in the dropdown menu, uncheck all boxes except for Forms so only forms are displayed. Also, double-check the Archive and Trash folders on the lower left side of the page to ensure the form is not accidentally moved there.
If the form is still missing despite these checks, please share any additional details about the error message or behavior you observe.

I see the screenshot of the issue you reported. But when I tested your form using the link, it shows it is active and available. See the screencast below for reference:
Could you try first clearing your browser cache, or use incognito or private mode browser, and try again using the link?
