Thanks for reaching out to Jotform Support. It seems the fields in your fillable PDF are missing because the Smart PDF Forms feature may not have detected or linked some fields automatically. You can manually adjust the field connections by mapping your online form fields to the PDF fields.
Here’s how to fix it:
- Open your form in Form Builder.
- Go to the PDF Editor. You can access this from the Publish tab by selecting the PDF tab and clicking Download, which redirects you to PDF Editor.
- Check the fields at the top of the original PDF. These appear as green boxes that you can drag, resize, and map to your online form fields.
- Make sure all required fields are properly connected. If you need to change field types, remove and re-add them in the online form to update the PDF.
- Save and download your updated fillable PDF.

Hi gbbventure,
Currently, there are widgets that are not yet supported in fillable PDF Forms, and the Dynamic Textbox widget is one of them. While the feature you’re looking for isn’t currently available at Jotform right now, we've gone ahead and escalated your request to our Developers. Exactly when or if it's developed depends on their workload, how viable it is, and how many other users also request it. If there are any updates, we’ll circle back to this thread and let you know.
In the meantime, you can consider using the basic Input Table element on your form. The only thing is that, it doesn't give your form fillers the option to add rows if they want to add more input. But, you can add multiple rows instead of just four. For example, you can add 10 rows for them. Let me walk you through it:
1. In Form Builder, click on Add Element in the upper-left corner of the page.
2. Under the Basic tab, select Input Table, and then click on its Gear icon on the right.
3. In the Input Table Properties panel, go to the Fields tab.
4. Select Textbox from the Input Type options.
5. Remove the second, third and fourth columns from the table by clicking on their X icon.
6. Add Row labels that you prefer. In my example, I put numbers 1 to 10.
7. Add your preferred Field label, and a Column label, like you see in my screencast below:

For the Dynamic Textbox widget that you have on your form, if you want to delete it, and replace it with the Input Table, I recommend downloading the submissions that you already received prior to this change. This is because removing a field or widget from the form will also delete its associated data. Let me show you how:
- On your My Tables page, hover over the form whose data you want to download, and click on View Table.
- On the upper-right side of the page, click the on Download All button.
- Now, select Download as CSV, Excel, or PDF to download your data.
