CNS FIELD TRIP FORM (D-142)
Please inform Child Nutrition Services (CNS) about the number of students who will be absent for breakfast or lunch whenever there is a field trip. This form must be completed and submitted to Child Nutrition at least TWO WEEKS BEFORE any scheduled event, including field trips, science camps, or any occasion when you anticipate a decrease in the number of students during breakfast or lunchtime.

CNS will provide bag lunches for any student who requests them. Under California Universal Meals, all students can receive these lunches for free, regardless of eligibility. Bag lunches typically include a sandwich, fruit, vegetables, milk, and a bottle of water, with a vegetarian option available upon request. Only order bag lunches for students who request them, as unclaimed lunches will incur a charge to the classroom.

Bag lunches can be picked up in the cafeteria before school. Rosters with instructions must be returned to CNS staff after distribution. These will ensure proper charging of lunch accounts and compliance with federal and state regulations.
