Steps to fill in the Online Sample Submission Acceptance Note
- Enter either your email address or your unique code in the block below
- Click on the ‘auto-complete fields’ button. Your company details should automatically fill. If not, kindly complete the form.
- Click to show the “Sample List” dropdown.
- Add your PO number, click the attach file(s) button, drag and drop your PO document, and enter your quote number.
- Fill in sample details, select tests and provide additional information (if applicable).
- Agree to FACTS Terms and Conditions.
- Click on the “Submit” button.
