  1. Go to the Jotform checklist maker page.

  2. Click the Go to My Forms button.
  3. Select Use Template.

  2. Find the Form element in your template. In this case, the element says Fill out the checklist.

  1. You can edit the form or replace it. To edit it, click on the Edit Form icon. Make any changes to the form, and then return to the App Builder.
