LiveWell is a dementia-positive organization and residential community that supports people with dementia. It offers prevention clinics as well as support in early stages of the condition through the later stages of cognitive change. For over 30 years, the Central Connecticut-based campus has championed comprehensive dementia care, providing community connection, support, advocacy, and resources.

“We offer everything from community classes all the way through assisted living services and skilled nursing services,” explains Maley Hunt, chief operating officer and administrator at LiveWell. “We even have a dementia-positive cafe on campus that is open to the public and staffed by volunteers who are experiencing cognitive change. We offer memberships to people in the community to use our amenities, like our gym and spa, different things that help people focus on their cognitive health and wellbeing.”

With all the resources and services LiveWell offers, staff needed a workflow management system that would support it all. After exploring other options, including Docusign and Adobe, the LiveWell team opted for Jotform Enterprise.

“We liked the flexibility, price point, and customizability for users, since we are a smaller organization,” says Hunt.

Simplifying signature collection

Prior to switching to Jotform Enterprise, LiveWell used hard-copy leasing agreements for residents, which were often a collection of papers so thick that they filled one-inch binders. When residents completed the leasing packets, they would have to mail or drop them off in person, which was time-consuming and challenging for those who didn’t live in the immediate area.

“Now, people can sign through Jotform Sign [Jotform’s e-signature tool] to complete their paperwork, which is really helpful for busy families and for people who might live further away, or if they have an attorney helping them complete the paperwork,” says Block. “It is also secure and HIPAA-friendly, which is really important in our line of work.”

“People are familiar with using Jotform Enterprise because you have so many customers,” says Hunt. “When they are signing important documents through this portal, they are already familiar with it because maybe their doctor’s office uses it or they have used it for other legal paperwork, like mortgages. There is a reputability and a comfortability with Jotform Enterprise.”

Managing events

LiveWell also uses Jotform Enterprise to manage requests for on-site events. With Jotform, staff can submit requests for events through a form that captures details like the desired room set-up. Another form collects food and beverage requests, and submissions are sent directly to the dining services team.

“We have both fixed settings and customizability within the form for what kinds of tables or setups you need,” says Hunt. “It might seem simple, but people don’t always think about all the questions that they’re going to need to put in. Being able to use Jotform really helps the event setup process.”

Before Jotform Enterprise, LiveWell relied on emails or even paper forms to plan events, which made it easier for details to get lost in the shuffle, leading to last-minute changes. Now, LiveWell can plan special events months in advance, delivering higher-quality experiences for their community members — as well as saving money.

“Now we are less likely to be running to the grocery store at the last minute,” says Hunt, laughing. “We can actually order ahead through our food purveyor and plan ahead for different events.”

Gathering data

Fostering community in both big and small ways is an essential part of LiveWell’s mission. For example, staff used Jotform to create personalized booklets about residents, allowing them to share details about themselves, answer questions, include pictures, and more. The team gathered the information for these booklets through a form. The booklets are available to anyone who goes to a particular “neighborhood,” like the Resilient Living Center, LiveWell’s community hub, which offers different classes and programs.

“It was really helpful because we created a QR code [for a form] and invited participants and their families to fill it out,” says Block.

LiveWell also uses custom Jotform forms for different classes they offer as well as their occupational therapy programs. As LiveWell continues to expand its programming, the LiveWell team will continue to expand its use of Jotform Enterprise.

“Jotform Enterprise has been easy to adopt, internally instrumental, and also very comfortable for our clients to use,” says Hunt.
