Innovative Care is dedicated to providing quality healthcare through modern technology. As self-described “early adopters,” staff of the Chicago-based medical group were offering telehealth visits long before the COVID-19 pandemic made them widely popular. Customizing forms for different patient types
Innovative Care offers a range of services, from primary and urgent care, to therapy, ADHD management, and weight loss. Specialty services, like ADHD treatment, use a custom appointment form that’s tailored to collect information for that particular treatment.
Whenever possible, Innovative Care allows patients to book appointments immediately online. But as a concierge-like practice, it also uses an intake form that allows patients to indicate what kind of care they are seeking and book a discovery call with an intake coordinator. For some practice areas, the team uses an intake form to collect information like past diagnoses, as in the case of its ADHD treatment services.

“Forms allow patients to sign up or express interest on their timeline,” says Monasteri. “If they are scrolling online late at night, they can reach out right then and there to get the ball rolling.”
For more unique service areas, such as Innovative Vitality, a service focusing on longevity medicine, the form includes a Calendly integration. “This allows the prospective patient to express interest and select a date and time they wish to speak with our intake coordinator to get more information or to book an appointment,” says Monasteri.
Automating appointment booking
Before adopting Jotform Enterprise, the Innovative Care team responded to each appointment inquiry manually through emails and phone calls. This made it harder for potential patients to book appointments, because it required back-and-forth communication confined to hours of operation. Now, patients can initiate care any time of day through forms, and conversion rates have increased significantly.
“When people are interested in signing up for something, they want to do it right away, and typically the more days pass, the less likely they are to sign up,” says Monasteri. “This is another example of how we’ve taken that extra step to connect with them and give them the answers they need in real time.”
For services that require a patient assessment, like ADHD services, the intake form is more complex and asks a series of qualifying health history questions. There is also a prompt to upload documentation of a previous medical diagnosis. Submissions to this form trigger a fully customized internal workflow, allowing the healthcare provider to review the patient’s data and proceed with the proper approach.
“If the new patient is approved, they get an email. If they aren’t, they get an email. It’s a very automated process,” says Monasteri.

This kind of customized automation is priceless for the Innovative Care team. “For a straightforward primary or urgent care appointment, you can simply book online. “Imagine, all of these would have required phone calls,” she says. “All of these phone calls would take 15 minutes each, and that’s a cost. You’d get busy signals and voicemails, and you’d be losing patients.”
