    We recently had an issue with our Canadian users about DNS but it has already been fixed. Can you try to open the form again and let us know how it goes?

    Coudl you let us know how you open the form? is it on desktop or in mobile? Can you try again in Incognito mode and let us know how it goes? This will help us to understand the issue better and investigate further. Also, I suggest clearing your browser caches. To do that you can follow this guide.
