How to create a form with multiple fill in blank fields

Thanks for reaching out to Jotform Support. We have available templates that you can use to start building your form. You may check our Free Online Form Templates page. If you already have a form ready, you can add the Fill in the Blanks element through the Form Builder. It's easy, let me show you how:

• In your Form Builder, click on Add Form Element + button at the top left of your screen.
• Under the Basic tab, drag and drop the Fill in the blank field to your form and that's it!

In addition, you can also use different fields inside your Fill in the blank field to meet the specific element needed. Simply click the + Element field under the Fill in the blank element.
