Hi there,
When we download our templated form it doesn't seem to pull the information efficiently and splits it up for 4-5 pages. We also need it to always pull "signature of the responsible party" at the bottom of the document even if it isn't signed so we can get our patients to sign it. This is very important. Thank you.

Just want to confirm, are you downloading the form directly from an email or from your Jotform account?

Thank you for the confirmation.
We cloned your form and tried test submissions then download the file thru PDF. We did not replicate the issue and we have all the details downloaded. Are you downloading the form first before you print them? You can try to download the form in formats of CVS, Excel of PDF.
We will wait for your response.
