Jotform Account Recovery

    The co-worker that created the account is no longer working for our company.

    I don't have the information to fill out the Jotform Account Recovery Form.

    I have a link to one of the forms that we have in our account. I've uploaded a screenshot of the form. That's about all the information I can find. I need help to recover our account.
