Now, coming back to your question, when I checked your form, the Confirmation Field option on the Email element is enabled which requires user's to input the email address twice. To fix the issue, you just need to disable it. Let me show you how:
1. In Form Builder, click on the Email field, then click on the Gear Icon.
2. Go to Options Tab, then click on Toggle switch to disable the Confirmation Field option.
