The document that is being pulled through once completed by our clients doesn't match what's going out. It is correct in the table, but not in the actual document. The final question isn't pulling through if the client selects 'I agree' and the prev multiple choice question is coming through wrong. When the doc is going out it looks perfect but not when it comes back completed

    Hi Brandon,

Thanks for reaching out to Jotform Support. Let me help you with the issue where the completed document does not match the original document sent out.

From what you described, the data in the table is correct but the final document is not pulling through some answers correctly, such as the final question when 'I agree' is selected and a previous multiple choice question showing incorrect data.

This issue often occurs when the PDF document template or the mapping between form fields and the PDF fields is not properly configured. To resolve this, please ensure that your Smart PDF form is correctly set up to map each form field to the corresponding field in the PDF. Double-check the field names and the conditions for the final question, especially for options like 'I agree'.

If you are using Jotform Smart PDF Forms, the original PDF layout should be retained with the submission data. You can also try downloading the original PDF with submission data from Jotform Tables or Inbox to verify if the data is appearing correctly there.

Here’s how to download the original PDF with submission data:

    1. Go to My Workspace page, select Tables, and click on View Table.
    2. Choose the entries you want to download.
    3. Click Download in the upper-right corner.
    4. Select Download as PDF.

If the problem persists, reviewing the PDF template settings or re-mapping the fields might be necessary.

Give it a try and let us know how it goes.

    Hi Brandon,

Are you referring to your Banter KING Non Exclusive Agreement form? If so, I checked it and noticed that the fields in question are not properly mapped to your original PDF. Take a look at the screencast below:
