Hi Ken,
Thanks for reaching out to Jotform Support. Let me help you with that:
To recreate the form you shared and edit it with your business information and logo, you can start by cloning the form in your Jotform account. Once cloned, you can customize the form by replacing the existing details with your business information and uploading your logo.
Here’s how to do it:
- Log in to your Jotform account and go to My Forms.
- Find the form you want to recreate or clone.
- Clone the form to create a copy in your account.
- Edit the cloned form by updating the business information fields as needed.
- Upload your business logo by adding an Image element or replacing the existing logo.
This way, you can have a customized version of the form tailored to your business.
Give it a try and let us know how it goes.

Hi Ken,
Cloning an Existing Form From a URL is easy. Let me walk you through it:
1. On your My Workspace page, click on Create on the top-left side of the page.
2. Select Form from the assets, and then click on Import Form.3. In the next window, click on From a Web Page.
4. Paste the form link into the Enter URL field and click on Create Form.Once we hear back from you, we can help you sort this out.

OK, I think I've cloned the form and edited it to my business specifics. Now what do I do with the "Submit" button to direct it to me via email I suspect ???
And, How do I get the number specific for this form so that when a customer clicks on the button in my webpage, it goes to my specific form ?

Hi Ken,
You don’t need to change the Submit button itself, it’s already set up to send the responses. To make sure submissions reach you by email, you need to set up an email notification in your form. This will send you an email every time someone submits the form. You can easily set up a Notification Email, let me show you how:
1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
2. In the menu on the left side of the page, click on Emails.
3. Now, click on Add Email.
4. In the list that comes up, select Notification Email.
5. In the center of the next window, click on the Recipients tab.
6. Now, just enter your email address into the Recipient Email field, and click on Save. That's it, you're done.
Give it a try and reach out again if you need any more help.
