Field added to published survey is not appearing

    I added a phone number field to a published survey - I can see it in the editing mode but it's not appearing in the preview or published modes. It's also showing up in the submissions report and the submission confirmation email template. I have no conditions set up and I've cleared JotForm's cache and my cache. Can someone please make it appear on the form?
