Add a fillable PDF to other page of a regular form

Hi,

Can JotForm create a registration form that captures name and demographics on one page, and add a fillable pdf to a second page. I want to capture some basic information from participants signing up for a training. They must also review and sign a consent form. Can the consent form be uploaded into the second page of the Jotform that allows me to add fillable fields for them to sign the consent form through the JotForm?

Unfortunately, it's not possible to add another fillable PDF to a second page of the same regular form. But you can redirect users to a second form that contains the uploaded Smart PDF Form with prefilled data, so they can continue and sign the PDF form. You can also send the link to this second form through the notification or autoresponder email to make the process seamless.
