Missing Company Name on Spreadsheet

The company name on the 2026 H2B Questionnaire is no longer appearing on the spreadsheet.

Please review the form settings to confirm that the company name field is still included and correctly configured to pass data into the spreadsheet. If the field was renamed, modified, or removed from the form, it would no longer populate in the spreadsheet export.

Yes, I changed the 1st question (company name) from short text to long text. How can i restore the form so that my spreadsheet reflects the company name's again?

Hi Melanie,

Because the Company Name field was changed from a Short Text field to a Long Text field, the connected spreadsheet may no longer reflect the data correctly. This happens because Jotform treats the modified field as a completely new one, which breaks the original mapping to the spreadsheet. Unfortunately, there is no direct way to simply convert the field type back to Short Text. To restore the original setup, the field would need to be recreated as a new Short Text field and then re-integrated with your spreadsheet.

Can you please provide step by step on how to change it back to short text and then reintegrate. Can you please tell me how many characters are allowed in short text?

Hi Melanie,

You can try to revert the form to a version where the change wasn't made. This is through the form's Revision History. You can use the Revision History feature that lets you revert your form back to an older version you select. Let me show you how to use it:

1. In Form Builder, click on the Circular Arrow icon at the top middle of the page to open the Revision History menu.
2. In the menu on the right side of the page, select the version you want to restore the form to. 
3. Now, just click on Revert next to the version you select, and then close the Revision History menu in the top right corner.

Let me take a look at this for you. To do that, we’ll need your permission to access your account. Don’t worry, you can turn it off again once we’re done. Let me show you how to do it:

1. In the top-right corner of your My Workspace page, click on your Avatar/Profile Image icon.

2. In the Dropdown menu that opens, click on Settings.

3. Next, in the panel on the left, click on the Security tab.

4. If you haven’t opened your Security page in a while, it’ll ask you to verify. To do that, click on Send Verification Code.

5. Then, enter the Verification Code you receive in your email.

6. Finally, at the bottom of the page, toggle the Jotform Support Access to the On position.

After we hear back from you, we’ll have a better idea of what’s going on and how to help.

Hi Melanie,

I got access to the form's Table and found out that 1. COMPLETE LEGAL BUSINESS NAME including DBA if applicable column is hidden. I've shared the steps below that you can follow to enable the hidden column:

1. On your Jotform Submissions Table, click on the Columns & AI dropdown button.
2. Scroll down and check the box on the missing column to show.
   

The column will show on the last column on the right side. You can drag the column by clicking and holding on the left side.
