    Hi Kim,

Thanks for reaching out to us for help.

Yes, you can upload your own forms to Jotform. Let me help you with that:

    1. Log in to your Jotform account.
    2. Go to My Forms where you can manage your forms.
    3. Click on Create Form and choose the option to import or upload your existing form file.

This way, you can easily use your own forms within Jotform.

Give it a try and let us know how it goes.

    Hi Kim,

In addition to Podo's answer, if you have your form in a PDF format, you can convert this to an online form using Jotform. Creating a form by uploading a PDF is easy. Let me show you how to do it:

    1. On your My Workspace page, click on Create on the top-left side of the page.
    2. Select Form from the assets, and then click on Smart PDF Form.
    3. In Smart PDF Forms, click on Upload Document, and then select the PDF you want to convert to a form, or just drag and drop the PDF into the white upload area.
