Yes, you can upload your own forms to Jotform. Let me help you with that:

Hi Kim,
In addition to Podo's answer, if you have your form in a PDF format, you can convert this to an online form using Jotform. Creating a form by uploading a PDF is easy. Let me show you how to do it:
On yourMy Workspace page, click on Create on the top-left side of the page.
Select Form from the assets, and then click on Smart PDF Form.
In Smart PDF Forms, click on Upload Document, and then select the PDF you want to convert to a form, or just drag and drop the PDF into the white upload area.
Sometimes the fields automatically created in the PDF file end up in the wrong place. If that happens, you’ll need to move and resize them. Check out the screencast below to see how it’s done:
If some fields aren’t added automatically, you can manually add them to your form, and then match it to the PDF on the right side of the page.
That's it. Once you set everything up, when the form is filled, it will automatically fill your PDF file. Check out the screencast below to see how it'll work:
